The supplies order form does not contain those documents which are available as free downloads from the CPC website. Although they are no longer on the form, they may still be obtained by special order for those who cannot get them from the website.
The National Supplies Order Form should be used for all book and general supplies orders.
The current version of this form is available for downloading as a PDF document or an MS Word document.
Branches may order badges using the special Badge Order Form which is available as either a Word document or a PDF document.
POSTAGE INCREASE
Postage rates have been increasing over the years. We can no longer absorb the increased cost of mailing.
Effective Jan.1, 2008, the minimum postage for orders will increase to $15.00.
We will continue our policy of issuing credits where 10% postage charges are excessive.
Also, due to lack of demand, we are no longer handling charms effective immediately.
